The sanatorium-resort card (Form 072-u) is a medical document that contains personal data and information about a patient’s health condition, existing diseases, contraindications, as well as specialist recommendations. Based on this information, the sanatorium doctor can prescribe appropriate treatment and nutrition.

For children traveling on a sanatorium package, a medical certificate in Form 076-u-04 is required.

The information contained in the sanatorium-resort card ensures an individualized approach for each patient, significantly increasing the effectiveness of treatment.

The document includes:

  • General patient information – full name, date of birth, contact details.
  • Medical history – chronic conditions, previous illnesses, and surgeries.
  • Examination results – laboratory tests, X-rays, and other diagnostic data.
  • Doctor’s recommendations – indications and contraindications for sanatorium treatment, including the necessary procedures.
Main steps for obtaining a sanatorium-resort card:
  1. Doctor’s appointment
    The first step is to visit a general practitioner or pediatrician if the card is being issued for a child.
  2. Medical examination
    During the appointment, the doctor will conduct an examination, review your complaints, and collect your medical history. Depending on your health condition, additional tests may be prescribed, such as blood tests, ultrasound, X-ray, etc.
  3. Filling out the card
    After all necessary information has been collected, the doctor will complete the sanatorium-resort card. It is important to provide the physician with as much detailed information about your health as possible to avoid potential complications during treatment at the sanatorium.

4. Obtaining the card
After completing all required examinations, you will receive the sanatorium-resort card in hand. In some cases, the document may be issued in electronic form.

5. Additional recommendations
After receiving the sanatorium-resort card, it is advisable to familiarize yourself with the treatment rules and conditions of the selected sanatorium, as well as clarify which additional documents may be required for check-in.

The Form 072-u certificate is valid for 60 days from the date of issue. It is recommended to start the process at least 10–14 days before the planned trip, as this is usually the time needed to complete all tests and obtain specialist consultations in a public clinic. The card can also be issued in a private medical center, provided it has the appropriate license.

When preparing for your trip to the sanatorium, do not forget your passport, SNILS number, and compulsory medical insurance policy, as well as your child’s birth certificate if traveling with children.

Please note that for stays under the “Wellness” program, a sanatorium-resort card is not required.
