What documents are required for a trip to a health resort in 2025
When planning your stay, it is important to prepare the necessary documents in advance. This helps avoid unnecessary stress and ensures a smooth check-in process. The list of documents depends on the guest’s age and the purpose of the visit.
For adults:
- Sanatorium-resort card (Form 072/u) issued no earlier than 2 months before arrival. This document confirms that the guest has no contraindications to the selected procedures and that the stay at the health resort will be beneficial for their health.
- Passport or another identity document
- Voucher or referral, if provided by an organization
- Compulsory health insurance (CHI) policy
For pregnant women:
- Maternity exchange card (prenatal record book)
For children:
- Sanatorium-resort card (Form 076/u) valid for 12 months from the date of issue. It can be prepared in advance, but it is important that the medical data (tests, specialist reports) are up to date by the arrival date.
- Birth certificate or passport (from 14 years old)
- Compulsory health insurance (CHI) policy
- Test for enterobiasis (pinworm infection)
- Dermatologist certificate confirming the absence of contagious skin diseases
Important: Sanatorium treatment is not provided for children under 4 years old. However, for accommodation with parents on the resort premises, the same set of documents is required, except for the sanatorium-resort card.
