A Health Resort Medical Record (Form 072/u) is a medical certificate required for admission to a health resort treatment program. It contains information about the guest's health condition, medical history, contraindications, and medical recommendations. Children participating in a health resort treatment program must provide Form 076/u-04.
The information contained in a Health Resort Medical Record helps ensure an individualized approach to each guest, significantly enhancing the effectiveness of treatment and wellness programs.
The document includes:
- Personal Information – full name, date of birth, and contact details.
- Medical History – chronic conditions, previous illnesses, and surgical procedures.
- Diagnostic Test Results – laboratory analyses, X-ray findings, and other diagnostic examination results.
- Physician's Recommendations – indications and contraindications for health resort treatment, including recommended medical and wellness procedures.
Steps to Obtain a Health Resort Medical Record
1. Schedule a Medical Appointment
The first step is to visit your general practitioner (GP) or family physician. If the medical record is being issued for a child, an appointment with a pediatrician is required.
2. Undergo a Medical Examination
During the appointment, the physician will conduct a medical examination, review your symptoms and medical history, and assess your overall health condition. Depending on your health status, additional diagnostic tests may be required, such as blood tests, ultrasound examinations, X-rays, or other medical procedures.
3. Completion of the Medical Record
Once all required medical information has been collected, the physician will complete the Health Resort Medical Record. It is important to provide accurate and comprehensive information about your health to help ensure safe and effective treatment during your stay.
4. Receive the Medical Record
After completion, the Health Resort Medical Record will be issued to you. In some cases, the document may be provided in electronic format.
5. Review Additional Requirements
Before traveling, it is recommended to familiarize yourself with the rules and treatment policies of your chosen health resort and confirm whether any additional documents are required for check-in.
The Health Resort Medical Record (Form 072/u) is valid for 60 days from the date of issue. It is recommended to begin the application process at least 10–14 days before your planned trip, as this is typically the time required to complete the necessary medical tests and obtain specialist consultations at a public healthcare facility.
The medical record may also be issued by a private medical center, provided it holds the appropriate license.
When traveling to the health resort, please remember to bring your passport, health insurance policy, and any other required identification documents. If you are traveling with children, please also bring the child's birth certificate.
Please note that a Health Resort Medical Record is not required for guests staying under the Wellness Program.
